What a proof of residency letter sample includes

A proof of residency letter — also called an affidavit of residence — confirms that a named person lives at a specific address. A complete sample shows:

• The resident's full name.
• The complete physical address, including unit, city, state, and ZIP.
• A clear statement that the person resides — or has resided — at that address.
• The dates the person has lived there, or a move-in date.
• The writer's relationship to the resident — landlord, property manager, family member, or employer.
• The writer's name, contact details, signature, and date.

Who can sign it, and when it must be notarized

The letter can be signed by anyone who can truthfully confirm where you live — most often a landlord or property manager, a family member you live with, or an employer. If you own your home, you can write a self-affidavit and support it with a utility bill or mortgage statement.

Some recipients — the DMV, courts, schools, and certain government offices — ask for a notarized proof of residency letter. In that case the signer signs in front of a notary public, who verifies their identity and stamps the document. Check the requesting office's rules before you submit, since a missing notary seal is one of the most common reasons a residency letter is rejected.
